Vejthani Hospital hereby announce that we will destroy documents regarding Patients’ Illness History, X-Ray Films, Heart-Ultrasound Result of the Out-Patient Department of Vejthani Hospital that have not been followed up with our Hospital for the duration exceeding 5 years since 2012.
In case the above announced items shall be needed, kindly contact Vejthani Hospital, telephone No. 0-2734-0000 extension Registration Department before 1st July, 2018, after this scheduled date all items shall be destroyed.
For acknowledgement, it is therefore announced.
